Now that you are ready to get on the road to effective weight loss, let us outline our intial signup process:

	Step 1: Medical and Consent Forms These forms give us an overview of your medical history and overall health prior to beginning the program. The consent form outlines your rights and expectations as a participant in the Slim Again hCG Protocol.
	Step 2: Laboratory Fee Once you complete your online form, the next step is take care of your lab fee. We exclusively use LabCorp because of their professionalism and number of laboratory locations throughout the nation. The fee for your lab work is $55. We use PayPal as our payment processor. If for any reason after we review your medical and consent forms, we determine you cannot participate in our weight loss program, your laboratory fee will be fully refunded.
	Step 3: LabCorp Assignment The final step is to locate a Labcorp location that is most convenient for you. With over 150 locations throughout the United States and Canada, we are confident that there will be one near you. If you live outside of North America, please contact us directly to make lab testing arrangements.
